SUPPLEMENTS BY SHAUN T – LIVE FITNESS EVENT
LIABILITY WAIVER, ASSUMPTION OF RISK & MEDIA RELEASE

By registering for and/or attending the live fitness event hosted by Supplements by Shaun T (the“Event”), I acknowledge and agree to the following:

ASSUMPTION OF RISK:I understand that participation in a live fitness event involves inherent risks, including but notlimited to physical exertion, muscle strains, sprains, falls, dehydration, dizziness, heartcomplications, or other injuries. I knowingly and voluntarily assume all risks, known and unknown,associated with participating in the Event.

MEDICAL ACKNOWLEDGMENT: I certify that I am physically able to participate in the Event and have no medical condition thatwould prevent safe participation. I understand that I am responsible for monitoring my ownphysical condition and agree to stop participating if I experience pain, discomfort, dizziness, orany concerning symptoms. I acknowledge that no medical services are guaranteed to beavailable on-site.

RELEASE OF LIABILITY:I hereby release, waive, discharge, and hold harmless Shaun T; Supplements by Shaun T; Shaun T,Inc.; all affiliated companies, partners, contractors, sponsors, instructors, employees, volunteers;the Event venue and its owners, operators, and staff; and any other associated parties(collectively, the “Released Parties”) from any and all claims, liabilities, demands, actions, orcauses of action arising out of or related to participation in the Event, including but not limited topersonal injury, illness, property damage, or death, whether caused by negligence or otherwise, tothe fullest extent permitted by law.

VENUE & PROPERTY DISCLAIMER: I understand that the Released Parties are not responsible for any lost, stolen, or damagedpersonal property before, during, or after the Event. I attend and participate at my own risk.

INDEMNIFICATION: I agree to indemnify and hold harmless the Released Parties from any claims, damages, losses,or expenses (including legal fees) arising from my participation in the Event or my breach of thisagreement.

MEDIA RELEASE & CONSENT: I acknowledge that photography, video recording, and audio recording will take place during theEvent. I grant the Released Parties the irrevocable right to capture, use, reproduce, distribute,display, and publish my image, likeness, voice, name, and/or appearance in photographs, video, oraudio recordings for marketing, advertising, promotional, educational, or commercial purposesacross all media platforms, worldwide, in perpetuity, without compensation or prior approval. Iwaive any right to inspect or approve the finished materials.

NO EXPECTATION OF PRIVACY: I understand that the Event is a public setting and I have no expectation of privacy while attendingor participating.

AGE REQUIREMENT: I certify that I am at least 18 years of age at the time of the Event. If required, I agree to presentvalid photo identification.

GOVERNING LAW: This agreement shall be governed by and interpreted in accordance with the laws of the state inwhich the Event is held.

SEVERABILITY: If any portion of this agreement is found to be invalid or unenforceable, the remaining provisionsshall remain in full force and effect.

ACKNOWLEDGMENT & ACCEPTANCE: I have read this waiver carefully, fully understand its contents, and sign it freely and voluntarily. Iunderstand that by agreeing to this waiver, I am giving up certain legal rights.
